In order to help us process your complaint as quickly and efficiently as possible can you please complete this form in as much detail as possible.
In the event that you have two or more faulty flavours please complete a separate Complaint Form for each one.
Please be aware that we can only replace devices purchased directly from us, all complaints are forwarded to the importers of these Puff Bars and we would also suggest that you contact them (details are on the Puff Bar boxes).
We would appreciate it if you can post your faulty device(s) back to us so that we can test them.
